سعر الاستشارة الطبية: مجانية – التطعيم/التلقيح ضد الحمى الصفراء: 19,46 اورو يجب دفعها بالبنك مع الاستمارات التي يقدمها المركز. بقية أنواع التطعيم الممسوح به للسفر: سعر موحد 36,85 اورو.
من المستحسن الذهاب الى المركز مسبقا على الاقل قبل شهر من السفر. ويجب تقديم للطبيب أكبر قدر ممكن من المعلومات حول السفر الذي سيتم القيام به: المناطق التي سيتم زيارتها ومدة السفر ونوعه.
ومن الضروري طلب موعد مسبق (باستثناء طلب اصدار نسخة مطابقة من شهادة التطعيم الدولية لأولئك الاشخاص الذين تم تطعيمهم بالمركز).
يتم طلب موعد لكل شخص يحتاج للذهاب الى زيارة الطبيب بغض النظر عن عمره وبالنسبة للسكان القصر الذين لا يتمتعون بوثيقة ثبوتية الهوية فان الموعد يتم طلبه باسم الاب او الام او الشخص الذي يمارس الوصاية عليه والشخص القاصر يجب ان يذهب الى العيادة بصحبة الاب او الام او الوصي. وللحصول على اي توضيح يرجى الاتصال بمركزنا.

شهادة الكوفيد الرقمية للاتحاد الاوروبي هي وثيقة اعتماد رقمية تثبت ان الشخص:
تم تلقيحه تطعيمه ضد الكوفيد-19 أو
تم اجراء فحص عليه يكون نتيجته سلبية او
قد تعافى من الكوفيد-19
السمات الرئيسية لشهادة الكوفيد الرقمية للاتحاد الأوروبي:
شكل رقمي او على ورق.
الاستجابة السريعة.
باللغة الوطنية والإنجليزية.
آمنة وموثوق بها.
شهادة التطعيم لها صلاحية سنة واحدة كأقصى تقدير.
ما هو نوع شهادة الكوفيد الرقمية التي يمكن ان تمد بها نقاط الاصدار الحضورية الخاصة بشهادات الكوفيد الرقمية؟
كل نقاط الاصدار الحضورية الخاصة بمقاطعة مدريد يمكنها اصدار شهادة الكوفيد الرقمية للمواطنين الذين يطلبونها تحديدا والذين يستوفون شروط الاصدار لكل شهادة كوفيد والمتعلقة بـ:
التطعيم والتعافي.
الاختبارات (تحليل بي سي ار والاختبارات السريعة للعنصر الجيني المضاد) التي تم اصدارها سابقا من طرف المختبرات او مركز مرخص له من طرف الهيئة الاستشارية للصحة بمقاطعه مدريد (طباعة شهادة معدة سابقا).
لأي شيء تصلح شهادة الكوفيد الرقمية للاتحاد الاوروبي؟
تصلح شهادة الكوفيد الرقمية لتسهيل التنقل بين الدول الاعضاء في الاتحاد الاوروبي وذلك للذين يثبتون احدى الشروط الثلاثة المذكورة (ان يكون مطعما/ ملقحا أوان يكون له اختبار سلبي أو انه تعافى من الكوفيد-19).
وبالتالي لا يعتبر هذه شهادة جواز سفر ولا وثيقة سفر يشترط أو يحد من حق حرية التنقل داخل الاتحاد الاوروبي وانما هو وسيلة تسمح في نفس الوقت تسهيل التنقل بين الدول الاعضاء وضمان الوقاية الصحية.
ورغم تمتعهم بشهادة كوفيد سارية المفعول وصالحة فإن كل المواطنين يجب ان يطلعوا على لوائح كل بلد ينتمي للاتحاد الاوروبي الذين سيسافرون اليه للتأكد من الشروط الخاصة بكل دولة عضوة حول قبول شهادات الكوفيد الرقمية.
وكما تمت الاشارة اليه في النقاط السابقة فعند سفر اي مواطن ينتمي للاتحاد الاوروبي او لبلد اخر يكون موجودا به او يقيم بصفة قانونية بالاتحاد الاوروبي ويكون بحوزته شهادة الكوفيد الرقمية فإنه يجب ان يكون معفى من وجود قيود تحد من حريته في التنقل بنفس الشكل التي تكون لمواطني الدول العضوة التي يتم زيارتها بيد انه يمكن ان تقوم احدى الدول العضوة ببعض التحويرات وتحدد الشهادات الموافق عليها وتفرض بعض الفحوص والاختبارات الاضافية او الحجر الصحي الخ. (انظر الى الشريحة الموالية) لكن لأجل ذلك يجب عليها دائما اعلام اللجنة وكل الدول الاخرى العضوة مسبقا وبإعطاء التبرير المناسب لقرارها.
للتمكن من الحصول على هذه المعلومات حول لوائح مختلف الدول بوجد صفحة ويب الاتحاد الاوروبي اين يمكن لاي مواطن تحديد بلده الاصلي والبلد الذي سيتوجه اليه لكي يعرف اللوائح الموجودة به قبل السفر.
كيف يتحصل المواطنون على الشهادة؟
السلطات الوطنية وفي هذه الحالة مقاطعة مدريد هي المسؤولة عن اصدار وختم الشهادة يمكن ان يكون اصداره مثلا عن طريق المستشفيات او المراكز التي تقوم بالاختبارات التشخيصية او السلطات الصحية والتي تكون مرخصة من طرف الهيئة الاستشارية للصحة بمقاطعة مدريد.
ويكون الاصدار عن طريق الورق او في شكل رقمي وكليهما يكون بهما رمز كيو ار الذي يحتوي على معلومات اساسية وكذلك يكون بهما ختم رقمي للتأكد من ان الشهادة أصلية.

وتضع مقاطعة مدريد في متناول المواطنين السبل التالية لتقديم طلب واصدار شهادات الكوفيد الرقمية (CCD):
عبر الانترنت: بتسليمها في شكل رقمي.
عبر البطاقة الطبية الافتراضية (ويبدأ العمل بها بداية من 15 يونيو 2021).
عن طريق بوابة المواطن الجديدة للقيام بإجراء شهادة الكوفيد الرقمية (ويبدأ العمل بها بداية من 22 يونيو 2021).

حضوري: بتسليم الشهادة على ورق وبإرسال رسالة على الهاتف اس ام اس للدخول الرقمي شريطة ان تكون مدرجة كل المعلومات المحدثة وذلك بمقاطعة مدريد.
وبالمراكز الصحية الخاصة والمختبرات المرخصة من طرف الهيئة الاستشارية للصحة لاصدار شهادة الكوفيد الخاصة بالفحوص التشخيصية التي يتم اجرائها (البي سي ار و الاختبار السريع للعنصر الجيني المضاد) بهذه المراكز.
